Reading advances your cursor; set `peek: true` to check without consuming. `limit` caps the list (default 50; overflow reported as `more`). The first call sets your watermark and returns no changes - but it still returns any `actions` already waiting for you, because outstanding work is not news you missed. Treat `actions` as an announcement and call list_actions for what you still owe.
