The caller must be a reviewer of every environment selected (the environment's reviewers, or the repository owner when none are listed). An approval runs the job (after the environment's wait timer, if any); a rejection fails it. Mirrors POST /api/v2/repos/.../actions/runs/:id/pending_deployments. Requires 'repo' scope.
